- PHOTO I.D FOR ALL ADULT FAMILY MEMBERS
- FEDERAL TAX RETURN FOR THE MOST RECENT YEAR (EVEN IF FILED JOINTLY)
- PROOF OF INCOME
- FOR SOCIAL SECURITY/SSI RECEPIENTS: MOST RECENT AWARD LETTER OR CURRENT PRINTOUT FROM THE SOCIAL SECURITY ADMINISTRATION
- FOR EMPLOYMENT: PLEASE PROVIDE LAST 3 MOST CURRENT, CONSECUTIVE PAY STUBS
- FOR UNEMPLOYMENT: PRINTOUT FROM THE UNEMPLOYMENT OFFICE AND 3 MOST RECENT, CONSECUTIVE CHECK STUBS.
- VERIFICATION OF CHILD SUPPORT (IF APPLICABLE)
- MOST CURRENT BANK STATEMENT
- IF YOU HAVE A PET, WE WILL NEED A COPY OF SHOT RECORDS AND SPAY/NEUTER RECORDS
